I’ve been using this script for awhile and it’s great. However, it is not working on my client’s site: http://aerialgrasshopper.com/contact.php

The form will go through and say everything sent, but my client has not received any emails.

What should I do?

The instruction to show the Subject seems clear:

$e_subject = ‘Contact from ’ . $name . ’ via your website.’;

There seem no obvious means to force the sender’s name in full, that is:

Jane Smith   Contact from Jane Smith via your website.

That’s correct. The form doesn’t do any validation on whether that’s a full or partial last name

Seems the email prefix is harvested and delivers it to the name showing in the intray.

Fatal error: Class ‘PDO’ not found

Since I can’t include the PDO option, I’ll just rewrite the script to work. I don’t remember reading anywhere that PDO was a requirement for installation…
While we use PDO on some projects, we don’t use PDO on this one (there’s no database). Based on the lack of a purchased icon, thinking you commented on the wrong product.

That can be done, but will require some PHP knowledge to make it work
Wow great form, but I’m lost. Does anyone have a tutorial on how to use a php form? I’ve built a simple PHP form, before but I’m lost with this one. Can you provide an HTML example?
nevermind I got it.
Purchased and implemented, the form is not working. It says mail successfully sent. When I tested it was working (on a different hosting), now when I am using it on Godaddy it isn’t.

Just checked Godaddy doesn’t support PHP Mail () Function, what should I do?
Its a question you’ll have to ask Godaddy to be honest. There should be no issues.
See if they can add it for you? Otherwise you’ll need to move hosts again!
Instead, can’t you help in adding a piece of code, where we can add the host name, username and password to send email.

got this form going instead of your ajax form. Ive styled it and instlled correctly. Validation works fine…however…i hit send and dont receive anything in my inbox?

Also how can i make the success messages appear in a popup instead of on the page?

Hey Stu,

The pop up would be custom code which we can’t support on here I’m afraid.

For the not receiving anything check the spam and the filters on the email provider. Thats the case 9/10.

Perhaps I’m totally wrong here, but why is the CSS NOT specific to just the form? Instead, your CSS file just has h1, h2 etc. (not .formh1). This affects all the H1s on my entire site.. even when including stylesheet.css (simply named stylesheet.css, instead of something like contactFormStyle.css). Seriously? So someone else comes along and there is this vaguely named “stylesheet”. That could be for anything! This, imho, this was not setup well for integration. But like I said, maybe I’m just wrong in my approach and understanding.

The script was intended to be a stand alone system so that its easier to showcase its functionality, hence why it have been created the way it has. Granted, the CSS could have been better created to make it easier for integration but it isn’t difficult to remove parts of the CSS or not include it at all and let your own CSS style the form.

Hi,

Apologies for the delay in replying. We’re having a little change in staff so there has been a little delay in answering comments and emails.

With regards to your question, unfortunately custom code changes are outside the scope of support provided, however, I can point you in the right direction.

You need to put the HTML markup for the new checkboxes in the form which starts on line 132 in contact_form.php. Then to get access to that values you need to put in an entry in PHP that handles the submit (line 32 onwards). Simply add

$checkbox = $_POST['checkboxname'];

and you will have the checkbox value. You might need to check if the checkbox exists in the POST first though.
